Ability to upload forms to fill out E.g. SCAT6

Feature Request

Currently I’m using Rehab Guru in a sports team, we have been able to make a SCAT form with in the forms feature but have come up against some challenges and had to find ways to format it in a way which still produces the same outcome - although the usage of the form now takes a little bit more thinking than the standard SCAT due to it working slightly differently. We’ve since gone back to using the paper version.

It would be helpful / beneficial to have a feature where we can upload a form such as the SCAT 6 and be able to edit it as a PDF with in Rehab Guru and save it to the patient’s file or something similar.

I recognise this is likely to be quite niche to my situation, however, if there are any other practitioners in a similar situation it may be quite beneficial.

    Hi Jack,

    Thanks for explaining this so clearly, and for giving the in-app SCAT form a go first.

    Uploading a PDF like the SCAT6 and filling it in directly inside Rehab Guru isn’t something we can do at the moment, so it’s logged here as a feature request for others in a similar setup to vote on.

    In the meantime, there are two ways to keep a completed SCAT6 on the patient’s record:

    Option 1: Upload it to their Files. Open the client, go to Files and click Upload a file (PDFs are fine). Our help article “Adding Files to a Client” walks you through it.

    Option 2: Attach it inside a treatment note. Add a question to your SCAT form and set its field type to File, so the completed PDF sits right alongside your notes. See “Adding Files to Treatment Notes”.

    If you’d like another go at the in-app version, the Matrix field type works well for the symptom grid, and a Formula field can add up the score for you. “Creating Patient-Reported Outcome Measure (PROMs) Templates” shows how. Or would it help if we created the SCAT6 form for you?

    To help us shape this, which parts of the SCAT6 were hardest to set up as a form?

      We’ve managed to set up the symptoms grid with the outcomes which is really helpful! The tricky part is the word recall where you have multiple trials, we’ve got a way that works, however when a patient is repeating words back to you it can be a bit tricky to keep up and complete the form correctly.

      I’m happy to share the form I’ve created and then maybe we could work on it and share it with other rehab guru users once its works smoothly?
