Thanks for raising this. Good news - you can already do it, using Smart Fields in Forms.
Smart Fields are form questions mapped directly to fields on the client record - name, date of birth, email, phone, address and so on. When the patient completes your consent form, their answers come back as proposed updates to their client details.
Go to Account > Content Management > Forms, then edit your consent form or select Add Template to build a new one.
Add a question and select a Smart Field as the question type. The + Add All control beside the Smart Fields (Client Details) heading adds every available field in one click, and there is an Auto-apply option if you would rather the client record update automatically.
When the completed form comes back, open Client Communication > Form Responses, preview the response, then choose which answers to apply to the client’s details.

One tip - preview the response before applying it, so a blank answer doesn’t overwrite something already saved on the record.
