Thanks for posting this. Good news on the first one: the PSFS is already in our sample forms library, ready for you to clone and use.
Step 1: Go to Account > Content Management > Forms.
Step 2: Open the Sample Forms tab, search for PSFS and click Clone. It will then appear under My Forms, where you can edit it and send it to clients like any of your other forms.
The GROC isn’t in the sample library yet, so I’ll work on bringing it into our library.
So we build the right one, could you let us know which version of the GROC your insurers ask for (for example the 7, 11 or 15 point scale), and whether they need any specific wording?
